NHPCO calls for applicants for the 2023 We Honor Veterans Outstanding Program Award

To celebrate the work of We Honor Veterans (WHV) partners, NHPCO is presenting the 2023 We Honor Veterans Outstanding Program Award to recognize the extraordinary investment, commitment, and achievement of hospice organizations providing high-quality, end-of-life care to Veterans and their families.

If you are a NHPCO member organization and a We Honor Veterans Level Four or Level Five hospice partner, you are invited to nominate yourself or another partner at the same level for the work that was accomplished during the last year with the program.

Apply for the Award

Who is eligible for the We Honor Veterans Outstanding Program Award?

We Honor Veterans Level Four and Five hospice partners and NHPCO member organizations are invited to nominate themselves for the work they have accomplished during the last year/their time with the program. All applications must be approved by organizational leadership (An Executive Director or CEO for example) and be received by the submission deadline to be considered for the award.

What is the nomination deadline for the We Honor Veterans Outstanding Program Award?

All applications are due by COB, August 24, 2023.

What is the application process for the We Honor Veterans Outstanding Program Award?

Submit your application via our online form by COB, August 24, 2023.

Supplementary materials for the application

You will have the option to include a short video at the end of this application form. Finalists may be asked to submit supplemental materials such as media articles, stories of Veteran patients, photos highlighting WHV activities, event fliers, and anything else the review committee should consider.

You will be required to confirm that by submitting your information, you are giving WHV and National Hospice and Palliative Care Organization (NHPCO) permission to post your pictures and your story on our social media accounts and within our blog pages, educational resources, and marketing and outreach activities and materials.

If you are submitting a photo or story with a patient, you must ensure that you have permission to do so and have filled out the proper release forms for your organization.

How is the award recipient selected?

The following criteria will be used to determine the award recipient:

  • Successful completion of the We Honor Veterans best practices:
    • Utilization of the Military History Checklist
    • Participation in a Hospice-Veteran Partnership
    • Implementation of a Veteran-to-Veteran volunteer program
    • Use of a post-death Veteran-specific satisfaction survey
  • Creativity in honoring Veterans
  • Staff engagement in Veteran program
  • Collaboration with local Department of Veterans Affairs
  • Local community engagement

How are the award applicants reviewed?

Applications will be reviewed by a committee of NHPCO staff (excluding the WHV team) and input will be provided by VA staff. Only the information provided in the award application will be used to determine the award recipient.

Applications should be completed with the assumption that the review committee has no prior knowledge of your Veterans program.

How will the award winner be recognized?

The winning organization will receive the 2023 We Honor Veterans Outstanding Program Award during the NHPCO 2023 Annual Leadership Conference at the Statehouse Convention Center in Little Rock, AR.

In addition to the award ceremony, the recipient will be recognized in various conference materials and communications. The winning organization will also be featured in a We Honor Veterans Quarterly Newsletter and in the 2023 Annual Report.

Please note: Award winners are expected to make their best effort to attend the conference and accept the award in-person.

Send in your nomination!

Nominate your organization for the 2023 We Honor Veterans Outstanding Program Award and let us recognize the work you’ve done support America’s Veterans.
