Reaching Out: Quality Hospice and Palliative Care for Rural and Homeless Veterans

Sept 2008 – Sept 2011

The Alliance executed a contract with the Department of Veterans Affairs (VA), to explore ways to improve access for homeless Veterans and Veterans in rural areas to quality end-of-life care. This initiative aimed to gain insight about the end-of-life care needs of these specific populations and subsequently implement community-based programs to address those needs.
