Join the Program

The mission of our partnerships is to establish an enduring network of care-at-home providers and Department of Veterans Affairs (VA) professionals, Veterans, volunteers, and other interested organizations working together to provide quality services across the care-at-home continuum for Veterans and their families.

Our partnerships provide leadership, technical assistance, and program development recommendations aimed at:

  • Improving Veterans’ access to care at home services across all sites and levels of care to ensure that every Veteran can receive care at the time and place of need;
  • Strengthening the relationships between community providers and VA facilities; and
  • Initiating comprehensive care and community engagement plans designed to reach Veterans.

About the We Honor Veterans Program Levels

As a We Honor Veterans Partner, there are three levels organizations who are Alliance Members can achieve. Each level guides you through specific activities using practical resources to progressively increase your ability to serve Veterans. Moving through the partner levels increases your organization’s knowledge and capacity to serve the Veterans in your community. It also demonstrates that your organization is making an active commitment to Veterans. When people in the community see that your organization is growing in the We Honor Veterans program, they’ll know that you can provide Veterans with compassionate care that is specific to their needs and goals.

As a Partner, Alliance members may promote their level of commitment to Veterans by displaying the We Honor Veterans Partner logo on their websites, as well as community outreach and educational materials.

The primary goals of each Partner level are:

Bronze Level: Commitment & Assessment

Establish baseline readiness and organizational capacity for Veteran-centered care by providing foundational education, identifying patients with military experience, and establishing practices that integrate Veteran-focused care into daily operations.

Silver Level: Implementation and Community Outreach

Integrate Veteran-focused practices into daily operations while expanding impact beyond the organization. This includes beginning community outreach activities and developing stronger relationships with Veteran-serving organizations.

Gold Level: Excellence & Leadership

Advance Veteran-centered care by improving access and quality of services while strengthening partnerships with community organizations and care-at-home providers, sharing knowledge, and mentoring peers.

How to Achieve Your Level

WHV Partners can achieve their level and a matching emblem by completing activities for each level of commitment. This enables VA staff and Veterans to easily identify care-at-home providers that have made a commitment to offer Veteran-specific care and services provided by a competent and highly skilled workforce.
