Level Five Requirements for WHV Hospice Partners

The goal of a Level 5 partner is expand & mentor: expand partnerships with other organizations in your community and other hospice providers to share knowledge and serve as a mentor.

WHV Hospice Partner Level 5 Requirements

Please review the requirements below to achieve Level 5 status.

Required Activities

  • All required Level 5 activities are listed in our Report Template.
  • Submit your completed Level 5 Activity Report in the Partner Portal. Use our Report Template to track your activities throughout the year.
  • Partners are required to complete Level 4 before beginning Level 5 activities.
  • Level 5 activities will only be accepted via portal submission. Email submissions will not be accepted.

Is NHPCO Membership required to achieve WHV Hospice Partner Level 5?

Membership with NHPCO is required to achieve Level 5. Please contact solutions@nhpco.org to confirm your active NHPCO membership.

If your hospice organization achieves Level 5 status, but does not renew its NHPCO membership, your organization will be re-ranked at Level 4.

Do I have to be a mentor to achieve Level 5?

Consenting to be listed as a mentor for lower-level WHV partners is a required component of Level 5. Partners pursuing Level 5 are asked to self-attest to possessing a firm understanding of the WHV program and being involved in the WHV program for one year or more. If your organization’s WHV primary contact does not meet these qualifications, another member of your organization may serve as your site’s mentor.


What is the Timeline for Completing Level 5?

All Level 5 activities must be completed within a 1-year period from committing to Level 5. Partners committed to attaining Level 5 will be assigned to either the April or October cohort, depending on their commitment date.

Partners in the April Cohort

  • Partners in the April Cohort must submit their completed Level 5 yearly on April 30.
  • Partners will receive confirmation of their report’s approval or disapproval by May 31.
  • When you being submitting your report through our Portal, you will have 90 days to complete it.

Partners in the October Cohort

  • Partners in the October Cohort must submit their completed Level 5 report yearly on October 31.
  • Partners will receive confirmation of their report’s approval or disapproval by Nov 30.
  • When you being submitting your report through our Portal, you will have 90 days to complete it.

To maintain Level 5 status, partners are required to recertify on a yearly basis. Please see our Level 5 Renewal Report Template.

If you do not complete Level 5 by your respective due date, your organization will remain at or be re-ranked to Level 4 status.

Please note: WHV will no longer grant any automatic due date extensions. In extreme circumstances, requests for extensions will be evaluated on a case-by-case basis.


What is the Level 5 Approval Process?

Upon receipt and approval of the completed Level 5 Activity Report, WHV staff will:

  • Provide your organization with a Level 5 Certificate of Recognition, Level 5 WHV logo, and Press Release template via email
  • List you as a Level 5 Regional Mentor (if applicable)
  • Update your hospice’s level on the WHV Partner Directory.
