Reaching Out Reports and Resources

From September 2008 to September 2011, The National Alliance for Care at Home executed a contract with the Department of Veterans Affairs (VA), to explore ways to improve access for homeless Veterans and Veterans in rural areas to quality end-of-life care. The initiative, entitled “Reaching Out: Quality Hospice and Palliative Care for Rural and Homeless Veterans,” aimed to gain insight about the end-of-life care needs of these specific populations and then to implement community-based programs to address those needs.
